Concierge Physical Therapy
We focus on high quality, physical therapy services for those who want to get more out of their rehab. Appointments are available in our clinic at Crossfit Stonefly during the hours available on our booking portal. We also have the option to travel to your home, for patients with difficulties leaving the house or even by telehealth follow ups to allow check ins with your therapist during bad weather days or long distance travel.
Which ever option suits you best, Dr. Cherwin has extensive experience helping patients of all ages and all abilities feel better, move better and live a higher quality of life.
Out-of-Network
In order to provide the service and rehabilitation treatments you deserve, Madison Valley Physical Therapy is out-of-network with private insurances. We believe "how many" and "what type of treatments" you receive should not be determined by insurance companies but instead by you and your therapist. Our services will be cash pay at the time of treatment.
High Deductible Plans
Do you know your deductible and co-pay for physical therapy? For many individuals with high deductible plans, attending physical therapy is not an option due to the high cost. Our goal is to make physical therapy more affordable for self-pay and high-deductible plan individuals. Superbills can be requested for self-submission to your insurance company.
The concierge structure allows you and your physical therapist to create a plan that works for your schedule with interventions designed to meet your goals. I hope to create convenience for your schedule, remove restrictions bound by insurance companies and allow quality one-on-one care without switching therapists, and eliminating double and tripple booking required by many private practices.
Do I Need a Referral?
No. Montana is a direct access state.
"Montana is one of 20 states that allow patients total, unrestricted access to a physical therapist. According to state law, no type of physician referral is required for a physical therapist to evaluate or treat a patient."
Can I use my FSA / HSA account?
Yes!
We are categorized as medical services and health practitioners, allowing you to use HSA and FSA cards for payment.
Do you accept Medicare?
Yes.
Madison Valley Physical Therapy is a contracted provider with Medicare part B. If you have Medicare, please book an appointment and upload your insurance card when prompted or bring it to your first appointment.
