Do you take my insurance?

We are in network with BlueCross BlueShield, Montana Healthy Kids, Pacific Source, TriWest and Medicare

All other appointments will be billed at our cash pay rate in which you are able to pay by cash, check, debit or credit card, FSA or HSA cards. Payment is due at time of service for all visits. If your insurnace company in not in netwrok with Madison Valley Physical Therapy, you are able to submit a claim with your insurance company to be reimbursed at out-of-network rates. We are happy to provide you with all necessary billing information. We are happy to help you with insurance matters, but it is up to you to understand your insurance coverage plan and deductible at the time of service.

High Deductible Plans

For many individuals with high deductible plans, attending physical therapy is not an option due to the high cost. Our goal is to make physical therapy more affordable. The concierge structure allows you and your physical therapist to create a plan that works for your schedule with interventions designed to meet your goals.

 

Cash Pay Rates

  • Initial Evaluation: $145 Evaluation

  • Follow up 30 minutes: $60

  • Follow up 45 minutes: $90

  • Follow up 60 minutes: $120

 

Can I use my FSA / HSA account?

Yes!

We are categorized as medical services and health practitioners, allowing you to use HSA and FSA cards for payment.

 

Do I Need a Referral?

No. Montana is a direct access state.

"Montana is one of 20 states that allow patients total, unrestricted access to a physical therapist. According to state law, no type of physician referral is required for a physical therapist to evaluate or treat a patient."

Do you accept Medicare?

Yes.

Madison Valley Physical Therapy is a contracted provider with Medicare part B. If you have Medicare, please book an appointment and upload your insurance card when prompted or bring it to your first appointment.

We are not contracted with Medicare Advantage plans. These are considered private insurance companies are not Medicare Part B. You are responsible for understanding your insurance coverage if you have a Medicare Advatage plan. You will be billed at the time of service and you are able to request the billing information to submit to your insurance company but it is your responsibility to understand your coverage.
